Black-and-white photographic print in a dark wood frame, headed "6TH ANNIVERSARY 2002" and branded "SHELBY AMERICAN COLLECTION" in the lower margin. The main image is the photograph known as "The Flying Mustang": Ken Miles airborne in GT350 5R002, car number 98B, at Green Valley Raceway, Texas on February 14 1965, with period spectators and bare trees behind. A smaller inset at lower left shows Miles at the wheel. The printed caption credits the photography to Robert Jackson and records that 5R002 was the original GT350 prototype and development car, later the Shelby American team car, and that it now resides permanently in the Shelby American Collection in Boulder, Colorado. Hand-inscribed in the lower margin: "For Charles, all the best, Bob Jackson 12/25/06".
